Patient Questionnaire Setup overview

The Patient Questionnaire module is designed to replace the new-patient forms, health history update, and other forms used in your office to collect information from patients. Patient Questionnaire allows you to create a custom set of questions that can be printed.

Once the patient completes a questionnaire, the answers can be entered into Dentrix Enterprise and stored as an electronic copy of the information. For those offices moving toward the "paperless office," the patient questionnaire is a way to computerize information that otherwise would be stored in a paper chart.

Also, when the information needs to be reviewed for accuracy, a new copy with the previous answers can be generated; the patient only has to modify those items that have changed since the last visit.

Note: Accessing the Patient Questionnaire Setup requires the "Patient Questionnaire Setup" security right.

To set up patient questionnaires

In Office Manager, on the Maintenance menu, point to Practice Setup, and then click Patient Questionnaire Setup.

If applicable, do one of the following:

If the Password - Patient Questionnaire Setup dialog box appears, user verification has been assigned to the "Patient Questionnaire Setup" task. Do one of the following:

If your user account has been granted the "Patient Questionnaire Setup" security right, enter your credentials in the User ID and Password boxes, and then click OK.

If your user account has not been granted the "Patient Questionnaire Setup" security right, have a user with permission temporarily override this restriction so you can access the Patient Questionnaire Setup this one time. The overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.

If a message appears, informing you that you do not have rights to this operation, your user account has not been granted the "Patient Questionnaire Setup" security right. To have a user with permission temporarily override this restriction so you can access the Patient Questionnaire Setup this one time, do the following:

On the message, click Yes.

In the Password - Patient Questionnaire Setup dialog box, the overriding user enters his or her credentials in the User ID and Password boxes and then clicks OK.

The Patient Questionnaire dialog box appears.

Perform any of the following tasks:

Create questionnaires.

Edit questionnaires.

Delete questionnaires.
